What is Support Brokerage?

Support brokerage is the process where a broker works with an individual or a team to set out goals and plans for their health and wellbeing, and finding ways to achieve and facilitate those goals and outcomes, using a collaborative, person-centred process and identifying and making use of all resources available. The broker also facilitates the individuals’ contribution to their own care, and enables creative thinking about how to holistically achieve (as far as is possible) the desired life for the individual. This can include working with a budget set by the NHS (via the local Clinical Commissioning Group or CCG), or Social Services (via the Local Authority) and building a package of care and support, as outlined in a personalised care and support plan, to meet the needs of that individual moving forwards with the funding allocated. This budget may also take note of other resources, such as benefits, grants, and other sources of income such as Access to Work. The support broker works closely with the individual – where able – and the family or circle of support, and ensures that they are central to the care and support provided.

Support Brokers work alongside people to support them to plan to live the life of their choice. They work directly for the person; supporting them to stay in control, empowering them and giving them the tools to have agency and autonomy and to live a life that makes sense to them. This involves exploring a wide range of support, resources, activities and opportunities to facilitate the goals and outcomes for that person.

Support Brokerage is built upon the tenet of people being in the “driving seat” of their lives, able to make their own choices and live a life that is fulfilling and enjoyable, and seeks to ensure the person is always at the centre of decision-making.

Areas of Expertise

I offer my services as a Support Broker with particular expertise in NHS Continuing Healthcare and Personal Health Budgets, but I work across the spectrum including social care personal budgets and direct payments, integrated budgets, and support brokerage for non-statutory support such as through the transition to adulthood, independent living, creating health passports, creating care and support plans across services and more.

Personal Health Budgets:

I have expertise around Personal Health Budgets and around especially complex needs. My experience includes setting up very complex packages of support; my own Personal Health Budget whereby I employ registered nurses (RNs) and carers (PAs)

Fees:

The costs for my service as a Support Broker are:

Initial assessment/contact call:

  • £30 for the first call to assess the support needed and whether it can be provided by myself, as well as to put in place agreements regarding my role, the scope of my role and the expectations of both parties.

Planning and developing package of support:

Identifying needs, sourcing support, support planning and building the support or care package:

  • £35 per hour for the services provided in respect of developing a support plan, mapping out needs, how to meet those needs, mapping out services and sources of support, and other services that need – or the client would like – to be involved. developing the budget, costing, identifying services and developing the support plan.

Implementing the plan and package:

This can be managed in two ways: a rolling-basis support, or a bulk purchase of support, and is as follows:

  • £40 per hour for support implementing the Support Plan and assisting with the support around the resulting care package;

Or

  • An upfront fee of £275 for up to 8 hours of support to assist with the implementation of the plan after the completion of the development and sign-off stage.

Other:

  • Information, Advice and Guidance session:

I can support with an information, advice and guidance session for people who just need to talk through support, ideas, services and tools and resources, rather than needing help with building a plan, support package or team. For this I normally allow a 2 hour session. Further sessions can be discussed and agreed.

£75 for 2 hours. 

  • Setting up a circle of support: 

I can assist with setting up a circle of support for an individual, and I can also support with circle of support meeting facilitation in the earlier days to get things settled in, with the view to another member of the circle taking over the facilitator role after 1-4 meetings.

£35 per hour.
